If you would like to save time, print out the New Patient Paperwork and have it filled out before you come in. The Adult Paperwork is for anyone 18 years of age or older. If you are suffering with neck and/or back pain, then you should fill out the neck and/or low back indexes as appropriate. If you are coming in for a wellness check, then you can leave those forms blank. For children under the age of 18, a parent can fill out the Pediatric Intake Forms.
